“证候”与“证”“候”的溯源与诠证

The Origination and Interpretation of "Syndrome", "Symptom" and "Sign"

  • 摘要: “证候”及“证”“候”, 是中医诊断学的基本概念、核心概念。但现代中医界对“证候”的解释不甚一致, 总体偏向于根据现代用法加以现代解释,而未能从这一概念的源起与其演变作动态的、立体的梳理, 特别是对“候”的认识偏差较大。梳理重要古医籍用例并与“证”“候”二字字义变化情况相互印证, 从动态演变的视角探求了二者的初始用法和引申变化, 从而得出“证候”的基本意义: “证”为病证, 指各别证状或证状群; “候”指人的体表对健康或疾病状态有提示意义的外在细微特征以及医者利用这些特征对身体、病情作出的评判。“证候”一词是此二者的综合。

     

    Abstract: "Syndrome", "symptom" and "sign" were basic and core conceptions in traditional Chinese medicine (TCM) while there was no definite interpretation of syndrome in the field of modern TCM, which generally tended to interpret it from the perspective of modern application. There was no dynamic and three-dimensional combing on its origin and development, especially for radically different understanding of "sign". The paper combed exemplifications in significant TCM classics, which was proved by the change of literal meaning of "symptom" and "sign". It explored the original usage and derivation change of "symptom" and "sign" from the perspective of dynamic change, thus the basic meaning of "syndrome" could be interpreted. "Symptom" referred to diseases pattern, including individual pattern or symptom clusters. "Sign" referred to the external subtle characteristics which could manifest personal health or disease situation and provide guidance for doctors to identify physical and disease condition. "Syndrome" was the combination of "symptom" and "sign".
